Adding an Intake Form from the Calendar

You can add an intake form directly from the calendar.  If you need someone to update their health information, or if you need to send out a specialized intake form like a COVID-19 Screening Form,  you can do it directly from the calendar. You can also send out agreements with your intake form.

  1. From your calendar, click on the Appointment
  2. Click Add Intake Form
  3. Select the appropriate Intake Form and Agreements (if applicable)
  4. Click Send
